A multi-ligamentous knee injury is a serious condition that results from high-impact trauma to the knee, such as a vehicle accident, fall from significant height, or blow that occurs while participating in a sport. This type of injury poses a significant risk to the limb as it can result in vascular damage as well. In addition, nerve damage often accompanies this type of injury, which can lead to sensory issues in the feet and ankles.
Multi-Ligamentous Knee Injuries Explained
The four ligaments in the knee are the medial collateral ligament, lateral collateral ligament, anterior cruciate ligament, and posterior cruciate ligament. These ligaments are essential for the proper function of the leg during activities such as walking and are often susceptible to injury when subjected to excessive force or impact.
While most knee ligament injuries only affect one ligament, severe trauma such as a direct blow to the knee can result in damage to multiple ligaments at once, which is known as a multi-ligamentous knee injury. This injury can lead to serious complications such as a disruption in the leg's blood supply and extensive nerve damage. Because of the nature of the trauma that leads to a multi-ligamentous injury, other structures in the knee may also be damaged and require surgery.
A Closer Look at the Ligaments in the Knee
MCL: The MCL keeps the inner knee stable and may become injured if the knee is hit from the inside.
LCL: This ligament can be found on the outside of the knee, also providing stability. Blows to the outer knee may result in a torn LCL.
ACL: The ACL is in the center of the knee. While anyone can sustain an ACL injury, they are common among those who regularly participate in sports. This is because ACL tears can result from directional changes, sudden stops, and twisting of the knee, which are often required in sports like basketball and soccer.
PCL: Behind the ACL sits the PCL. If the knee receives a blow while it is in a bent position, a PCL tear can result.
Damage to multiple ligaments accompanies symptoms such as severe pain, weakness in the joint, and a limited range of motion.
Treating a Multi-Ligamentous Knee Injury
Surgery is needed to reconstruct the ligaments and restore stability and is usually performed shortly after the knee injury is sustained. Often, grafts are required for successful treatment. It is possible for all damaged ligaments to be repaired in one surgery, but some cases may require multiple operations. If it is determined that there is damage to the artery, emergency surgery may be required to restore blood flow.
